Carlo DiClemente, supervisore dei percorsi di Formazione a Distanza di Luoghi di Prevenzione, è professore di Psicologia della UMBC (Università di Baltimora), dove dirige il laboratorio “Habits”, per lo studio del cambiamento dei comportamenti disadattivi. E’ direttore di diversi progetti di ricerca ed intervento, in particolare per la cessazione dal fumo di sigaretta (MDQUIT) e per l’organizzazione motivazionale dei servizi sanitari. Insieme a James Prochaska ha fondato il Modello Transteorico del cambiamento (TTM).

Questo modello si propone di raccogliere i contributi scientifici di diversi orientamenti per definire una teoria generale della motivazione al cambiamento. In questo modello la motivazione non è un tutto o nulla: varia da una completa assenza sino all’iniziale dubbio se cambiare o meno, per giungere alla determinazione di cambiare, e quindi operare dei primi cambiamenti ed infine mantenerli nel tempo. Questi sono gli stadi del cambiamento: precontemplazione, contemplazione, determinazione, azione e mantenimento. Il modello transteorico propone però una descrizione molto più ricca. Prevede due indicatori del cambiamento: in primo luogo la bilancia decisionale, che misura i pro ed i contro che la persona attribuisce al cambiamento; e successivamente l’autoefficacia, intesa come la fiducia espressa dalla persona a poter superare i momenti di tentazione. Infine descrive una serie di processi del cambiamento, che sono come dei compiti che la persona deve progressivamente eseguire per maturare la decisione di cambiare e mantenerla nel tempo.

Lo scopo principale del modello transteorico è quello di stimolare l’operatore sanitario a guardare con occhio esperto la persona che deve cambiare un comportamento. E quindi saper aiutare la persona a procedere nella propria motivazione facendo la cosa giusta al momento giusto; evitando di forzare la mano con le persone che non sono ancora pronte, ma anche accompagnando chi ha già attuato un cambiamento in modo che lo mantenga nel tempo. La persona che cambia non è una lampadina fulminata. E sarà sempre più motivata a cambiare se l’operatore della salute saprà starle accanto con umanità e competenza.
